BLS changed its injury classification system starting with 2023 data, so the 2011-2017 forklift figures are shown as their own historical block and are not compared arithmetically with 2023-2024 figures. Employer-reported BLS data undercount injuries that are never recorded; OSHA-reported Amazon figures come from the company's own filings. Confirm any figure against the specific source before relying on it.

How often warehouse workers get hurt
According to the Bureau of Labor Statistics' 2024 Survey of Occupational Injuries and Illnesses (bls.gov), warehousing and storage employers reported 77,000 nonfatal injuries and illnesses, a rate of 4.8 per 100 full-time workers against 2.3 for all private industry. The gap is wider on the serious end: a DART rate of 4.1 against 1.4, and a median of 15 days off work per days-away case against 8. According to OSHA's 2026 warehousing emphasis directive (osha.gov), the five-year 2020-2024 average for general warehousing was 5.2 recordable cases and 4.4 DART per 100 workers, and the industry's workforce more than doubled to 1,836,200 between December 2015 and December 2025.

What this means: the warehouse injury problem is a frequency problem and a severity problem at the same time. A facility running at the industry average will log about one recordable injury for every 21 full-time workers a year, and the median days-away case keeps that person out for 15 days. For anyone managing or insuring the building, the 2.6-point DJTR rate is the number to watch: most of the excess is restricted duty and job transfer, meaning the typical warehouse injury is one the worker returns from but cannot do the full job after.
What causes the injuries
Forklifts get the headlines, but they are not the main way warehouse workers get hurt. According to the Bureau of Labor Statistics' 2023-2024 case and demographic tables (bls.gov), overexertion and bodily reaction caused 23,100 of warehousing's 53,090 days-away injuries, 44 percent of the total and more than three times the all-industry rate. Contact with objects and equipment caused 27 percent, including 4,860 workers struck by falling, collapsing or suspended objects, the code that captures racking, pallet and load failures. Falls, slips and trips caused 18 percent, most on the same level. Transportation incidents, the code that holds forklift collisions and overturns, caused 8 percent, but at almost three times the all-industry rate. Struck-by, same-level falls and running-equipment contacts are the categories where floor and rack condition matter most: blocked aisles, damaged uprights, loads stacked past the beam rating, worn dock plates, faded floor markings.

What this means: the occupation table settles the "who" question. A hand laborer or freight mover is injured at nearly four times the all-occupation rate and a forklift operator at slightly below it, which is why an ergonomics program pays back faster than another operator refresher. The struck-by and same-level fall categories, about 11,100 cases together, are the ones a walk-through photo of the aisles, racking and dock catches before an inspector or an incident does. Forklift injuries and deaths, the current figures
The current forklift count is 84 deaths. According to the National Safety Council's Injury Facts (injuryfacts.nsc.org), which republishes BLS fatality and injury data, forklifts were the source of 84 work-related deaths in 2024 and 25,110 DART injuries over 2023-2024, with a median of 22 DART days per case. According to the BLS Census of Fatal Occupational Injuries (bls.gov), industrial truck and tractor operators, the occupation code for full-time forklift drivers, suffered 30 fatal injuries of all kinds in 2024; forklift deaths among laborers, farm and construction workers who drive occasionally are coded to their own occupations. The warehousing and storage industry recorded 32 fatal injuries in 2024. The last time BLS published a dedicated forklift fact sheet, in June 2019, it counted 614 forklift deaths over 2011-2017 and more than 7,000 days-away forklift injuries a year, with pedestrian injuries carrying the longest recovery.

What this means: the forklift death count sat between 74 and 94 in every year of the 2011-2017 BLS series, and the 2024 figure of 84 is inside that band. The split matters more than the total: in the last BLS breakdown (2017), nonroadway vehicle incidents, mostly overturns, were the largest fatal share at 27 percent, pedestrians 12 percent and falling objects 16 percent, while nonfatal injuries skewed toward pedestrians (20 percent). Pedestrian separation, load stability and rack condition are the levers, and all three are visible conditions.
The recycled forklift numbers, traced to their source
Many forklift training and certification pages repeat the same four numbers: 85 deaths a year, 34,900 serious injuries, 11 percent of forklifts in an accident each year, 70 percent of accidents preventable. We traced all four. According to OSHA's March 14, 1995 proposed operator-training rule in the Federal Register (osha.gov), forklift accidents caused "approximately 85 fatalities and 34,900 serious injuries each year," an estimate built on 1981-1990 averages of 107 deaths, 33,800 serious and 61,800 non-serious injuries. The same document divides 68,400 accidents by 855,900 trucks and calls the result "approximately 8 percent," not 11. Its 70 percent is a reduction in operator error rate from the Cohen and Jensen training studies, which OSHA said "can not be directly equated" to fewer accidents. According to OSHA's December 1, 1998 final rule, the agency replaced the estimate with 101 fatalities and 94,570 injuries a year, counted 998,671 trucks, and settled on 25 percent of accidents as preventable by training. NIOSH's 2001 alert (cdc.gov) is the source of the most-quoted cause share: overturns, about 25 percent of forklift deaths.

According to Safety+Health magazine's coverage of OSHA's preliminary fiscal 2025 list (safetyandhealthmagazine.com), powered industrial trucks (29 CFR 1910.178) was the 8th most-cited standard with 1,826 violations, down from 6th and 2,248 in fiscal 2024. General warehousing and storage was the industry most often cited under it. The single most-cited clause was 1910.178(l)(1), safe operation and training, at 413 violations. According to the Department of Labor's penalty rule in the Federal Register (federalregister.gov), the maximum is $16,550 per serious violation and $165,514 per willful or repeated one, unchanged for 2026 because the missing October 2025 CPI cancelled the annual adjustment. According to OSHA's 2026 directive (osha.gov), the warehousing emphasis program found more than 1,700 violations in its first 18 months and was reissued on July 31, 2026 for five years. The standard itself requires a truck examination at least daily and an operator evaluation at least every three years.

Amazon and the largest warehouse employers
One employer shapes the national warehouse numbers. According to the Strategic Organizing Center's May 2025 analysis of Amazon's OSHA 300A filings (thesoc.org), Amazon's warehouses had a serious injury rate of 5.9 per 100 workers in 2024 against 3.0 at non-Amazon warehouses. The company employed 39 percent of U.S. warehouse workers but accounted for 56 percent of serious injuries, and it reported 39,062 recordable injuries across 1,172 facilities. According to the National Employment Law Project (nelp.org), Amazon operates 168 of the 253 U.S. warehouses with more than 1,000 employees. Amazon's own account (aboutamazon.com) is that its recordable incident rate improved 43 percent and its lost-time rate 70 percent over six years. According to the Senate HELP Committee's interim report (help.senate.gov), Amazon's internal data put its Prime Day 2019 recordable rate above 10 per 100 workers.

What this means: two things are true at once. Amazon's rates have fallen by its own and by the SOC's numbers, and they remain roughly double the rest of the sector's. Because Amazon holds 79 percent of employment at large warehouses, any "industry average" for big facilities is mostly an Amazon number; benchmark a non-Amazon operation against the SOC's 3.0 serious-injury rate or the BLS industry-wide DART rate of 4.1 (serious cases on both measures), not against the large-establishment average.
What the injuries cost, and the fleet behind them
According to Liberty Mutual's 2025 Workplace Safety Index (libertymutualgroup.com), serious disabling injuries cost U.S. employers $58.78 billion a year, and the two causes that dominate warehousing, overexertion from handling objects ($13.7 billion, "largely due to manual material handling") and same-level falls ($10.5 billion), are the two most expensive nationally. According to Travelers' 2026 Injury Impact Report (travelers.com), an injured worker now misses an average of 80 workdays, and first-year employees, the profile of a peak-season warehouse hire, account for about 37 percent of injuries. The equipment side keeps growing: according to the Industrial Truck Association's president writing in DC Velocity (dcvelocity.com), North America recorded over 195,000 forklift retail orders in 2024, 71 percent of them electric.

What this means: the warehouse's two biggest injury causes are also the country's two most expensive, and the workers most likely to be hurt are the ones hired last month. Claims we could not verify
Four numbers circulate widely without a primary source we could locate. We list them so writers can stop repeating them.
- "11 percent of forklifts are involved in an accident each year." Not in either OSHA rulemaking. The 1995 proposal's own arithmetic (68,400 accidents divided by 855,900 trucks) comes to "approximately 8 percent." We found no document that says 11.
- "70 percent of forklift accidents are preventable." The 70 percent is an operator error-rate reduction in the Cohen and Jensen studies as OSHA cited them in 1995. OSHA said it cannot be equated to accident reduction and used 25 percent in the 1998 final rule.
- "OSHA ran 623 warehousing inspections and proposed $2.4 million in penalties in the program's first seven months." Repeated by insurance and safety blogs in 2024. We found no OSHA or Labor Department document stating it; OSHA's 2026 directive reports 18-month totals instead (more than 1,700 violations, about 37,410 workers removed from hazards).
- Racking collapse counts. No national count of pallet-rack collapses exists in the BLS, OSHA or NIOSH data we could fetch. BLS folds rack failures into "struck by falling, collapsing or suspended objects," 4,860 warehousing days-away cases over 2023-2024. Any page giving a precise annual number of rack collapses is estimating.

84 in 2024. The National Safety Council's Injury Facts, using BLS Census of Fatal Occupational Injuries data, reports forklifts were the source of 84 work-related deaths in 2024. BLS counted 614 forklift deaths from 2011 to 2017, about 88 a year. The often-quoted 85 deaths a year is a 1995 OSHA estimate based on 1981-1990 data.
What is the warehouse injury rate per 100 workers?
4.8 recordable cases per 100 full-time workers in 2024 for warehousing and storage (NAICS 4931), per the Bureau of Labor Statistics, versus 2.3 for all private industry. The DART rate was 4.1 versus 1.4. OSHA's 2026 warehousing directive puts the 2020-2024 five-year average for general warehousing at 5.2 recordable and 4.4 DART.

Forklifts were the source of 25,110 DART injuries over 2023 and 2024 combined, 15,460 of them days-away cases, per the National Safety Council's Injury Facts using BLS data. That is roughly 7,700 days-away forklift injuries a year. BLS counted 9,050 days-away forklift injuries in 2017 alone. The 34,900 serious injuries figure is a 1995 OSHA estimate.
What is the most common cause of warehouse injuries?
Overexertion and bodily reaction, at 23,100 of the 53,090 days-away injuries in warehousing over 2023-2024, or 44 percent, per BLS Table R4. Contact with objects and equipment caused 14,590 (27 percent), falls, slips and trips 9,600 (18 percent) and transportation incidents 4,050 (8 percent).
Is powered industrial trucks still an OSHA top 10 violation?
Yes. 29 CFR 1910.178 ranked 8th in fiscal 2025 with 1,826 violations, down from 6th and 2,248 in fiscal 2024, per Safety+Health magazine's coverage of OSHA's preliminary list. The most-cited clause was 1910.178(l)(1) on safe operation and training, at 413 violations, and general warehousing and storage was the most-cited industry under the standard.
