Failure rates: what inspections actually find
The benchmark numbers come from the UK's Fire Door Inspection Scheme, reported by The Construction Index (theconstructionindex.co.uk): of more than 100,000 fire doors inspected by approved inspectors in 2021, 75 percent failed to meet required standards, barely improved from the 76 percent it found in 2019, and 31 percent were never fit for purpose in the first place. The US has no equivalent survey, but the reported experience matches: inspectors report 75 to 90 percent failure on first inspections, per Facilities Management Advisor (facilitiesmanagementadvisor.com). The rules and the cost of the annual inspection
The inspection requirement is written into NFPA 80: fire door assemblies must be inspected and tested annually, plus after installation and after maintenance work, against the visual criteria of paragraph 5.2.4.2, 11 items covering labels, clearances, closers, latching, and field modifications, per Health Facilities Management (hfmmagazine.com). The Steel Door Institute (steeldoor.org) publishes the same paragraph as a 13-point checklist and adds the recordkeeping rule: inspection records kept 3 years, acceptance test records for the life of the assembly, deficiencies corrected without delay.
